For instance, if I right clicked on a word in a document and wanted to add the command to “open new Comment” to the right-click menu… could I add that? Is any customization possible?

Sorry, but that level of customisation is not available in Scrivener. Do note there are keyboard shortcuts for inserting comments and annotations, however. Note you can also click the “+” button in the Comments Inspector pane, too.
