Make sure you’ve selected “Title” in the list of elements to be included.
In Scrivener 2, it’s the Compile -> Formatting pane. In Scrivener 3, edit the Section Layout for the affected documents.

Thanks for the reply. I need to clarify and provide more details:
I’m using Scrivener for PC version: 1.9.7.0
I ticked pretty much everything trying to get it to output a Word .docx with chapter headings and the scenes.
Each chapter is a folder labelled Chapter 1, Chapter 2, etc.; inside each folder are text documents containing one scene each.
In the Scrivener Contents panel I have Title and all chapter folders and scenes ticked in the Include column (Have them set for As Is).
In the Formatting panel, I have Title ticked for all three Types: folders and both items that look like text documents (All three items show as Level +).
Unless I’m missing something, this should produce a compiled output to Word with both the Chapter titles and the scenes showing, but only the scenes appear in word, no Chapter headings at all.
Did I miss something?
I have the same problem. I find this reply utterly baffling, as the options I get are extremely difficult to follow. Is there a STEP-BY-STEP instruction on adding in chapter titles?

In v3 (win beta 2.9.x.x) click the “assign section layouts” button – that will allow you to select chapter names, numbers, etc.
Be prepared for a bit of trial and error in compile!
The answers are going to be very different for the current release for WIndows vs. the current beta for version 3. The solutions are going to vary greatly because there are a variety of reasons why titles might not show up, so one person’s solution may not work for you.
So you need to provide at least those details, and it’s better to open your own threads. Screenshots of your compile window, showing the list of documents to be compiled + the options for including binder titles can also be very helpful.
That said, one way for titles to be suppressed in the old (version 1) Windows Scrivener is to check the “as-is” compile option for items in the binder.
