Document icon on top of chapter icon?

The novel is done and a pdf gets sent to the agent tomorrow. There’s just one little nagging thing that’s not clear. I have my Binder arranged in Parts, Chapters, and Scenes. The parts and chapters are represented by folder and the scenes are represented by document icons.
Everything looks great except for chapter 35. The chapter has a document icon superimposed on it. How did it get there and what does it mean?

You can enter text in the Editor for a folder separate from its role as parent/chapter. Good luck with your submission.
