Document icons in binder view

I don’t understand the difference between the following icons. Nor how to create each of them.

1). The folder which is a plain looking folder. I know how to create this.
2). In the tutorial, there is a folder with a text icon on top of it. I don’t know what this is, nor how to create it.
3). In a previous document, I had apparently learned something from the tutorial or something else I have the following icons that I don’t know what they mean, nor how I got them.
a). a text icon with lines on it that displays one sheet on top of the other.
b). a text icon that is blank but also displays one sheet on top of another.

Please see the section on “The Binder” in the Help file (select “Scrivener Help” from the Help menu inside Scrivener, click on “Reference Guide”, and then click on “The Binder” in the left hand list of topics).
