Headers & Footers Not Showing Up In Compile to PDF Paperback (6" x 9")

When I compile to PDF using the Paperback (6" x 9") format the headers and footers are missing. However, if I compile to PDF using the Paperback (5.06" x 7.81") format the headers and footers do appear.
