How do I insert a document header?

I’m using the Beta provided for NaNoWriMo participants. The manuscript sample shows a document header in use (Surname/TITLE/2) along the top.

I cannot find how to insert a header into my document (or a footer for that matter). I’m probably blind. Possibly Scrivener doesn’t do this until the document is compiled.

It’s driving me a little nuts. Could someone clue me in? Thanks!

(Also, I just tried the find/replace function on the sample manuscript and while the popup menu allows me to type in a word, nothing happens when I click on any of the options.)

Yes, the header is set and is placed at Compile time.
