It was a passing remark previously but without thinking I tried to use “additional substitutions” again on an index card and nothing happened - is that a bug (to be fixed hopefully!) or by design for some obscure reason? (In which case, why is it not appropriate here?)

This isn’t technically a bug, but rather just something that isn’t implemented. I’ve added it to the list of suggestions though, as it makes perfectly good sense to me to have these available not only in index cards, but custom metadata and title fields.
Ah. I thought it would be something that acted wherever text could be typed… understood!
