File folder meaning

I’m flummoxed. I know there’s a simple solution tho this issue, but I can’t find it in the Manual.

In the novel format, I treat chapters as folders and scenes w/in chapters as documents/files. On some folders, there’s a small document showing, but on most, no such small document. See the attached. “Loose Ends” and “Phoenix” are regular folders; “A clear and Straight Course” has the little document flag. As far as I can tell, the folders are identical. What am I missing? How can I make all folders like the normal form (e.g., “Loose Ends”)?

The icon on that one folder shows that there’s some text in its index card.

The small index card badge means this folder has a synopsis added to it.
